Screw-retained is the default for posterior cases, multi-unit bridges, deep margins where cement removal is risky, and any case where future retrievability matters. Choose cement-retained for anterior esthetics with shallow margins and prosthetic angle that won't accommodate a screw access channel.
Two paths: (a) use an angulated screw-channel system (compatible with major implant systems) to redirect the access lingually, or (b) switch to cement-retained. Indicate the issue on the Rx with photos and we'll advise.
Modern monolithic ProXzir® designs maintain structural integrity around the screw access; the access is sealed with composite after final torque. For high-load posterior cases, prescribe SS1400 for maximum fracture resistance.
